A Change Request Template provides a structured way to document, assess, and approve proposed changes to a project’s scope, schedule, budget, resources, or requirements. It helps ensure each request is clearly understood, its impacts are considered, and decisions are recorded before work proceeds.
To use it, describe the proposed change and why it is needed, then note the affected deliverables, schedule, costs, resources, and risks. Identify the person requesting it and any supporting information, and submit the form to the designated project lead or change control group for review.
Record the decision, approval details, and any conditions; if approved, update the relevant project plans and communicate the change to affected stakeholders.
The template is and Excel workbook and compatible with Google Worksheets. A download link will be automatically emailed to you once the payment has been completed.
